Job summary

Sangre de Cristo Community Care - Hospice in Pueblo, Colorado seeks a compassionate Certified Nursing Assistant to join our hospice home care team. You will provide one-on-one care to patients in their homes, supporting comfort, dignity, and quality of life.

This weekend day position requires strong communication, reliable transportation, and the ability to work with an interdisciplinary team to implement each patient's Plan of Care.

Qualifications

• Current Colorado CNA certification in good standing.
• At least one year of CNA experience preferred.
• Hospice/home health or other direct patient care experience preferred.
• Valid driver's license and reliable transportation.
• Compassionate, dependable and comfortable providing end-of-life care.
• Strong communication and time-management skills.
• Ability to work independently while remaining connected to the clinical team.
• Availability to work Fri–Sun day schedule, including holidays.

Responsibilities

• Provide personal care including bathing, grooming, dressing and toileting.
• Assist with mobility, transfers, positioning and ambulation.
• Provide assistance with meals, hydration and nutritional needs.
• Observe patients and promptly report changes in condition to the nursing team.
• Help maintain a safe, clean and comfortable environment for patients.

Sangre de Cristo Community Care is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our hospice home care team in Pueblo, Colorado.

As a Hospice CNA, you'll have the opportunity to provide meaningful, one-on-one care to patients in the comfort of their own homes. You'll be an important part of an interdisciplinary hospice team focused on helping patients live with comfort, dignity and quality of life.

This position works Friday, Saturday and Sunday during the day.

What You'll Do

Working under the direction of an RN or Case Manager and following each patient's individualized Plan of Care, you will:

• Provide personal care including bathing, grooming, dressing and toileting
• Assist with mobility, transfers, positioning and ambulation
• Provide assistance with meals, hydration and nutritional needs
• Observe patients and promptly report changes in condition to the nursing team
• Help maintain a safe, clean and comfortable environment for patients
