Job summary

Sangre de Cristo Community Care in Pueblo, Colorado is seeking a compassionate Certified Nursing Assistant (CNA) to join our hospice home care team. As a Hospice CNA, you will provide one-on-one care in patients' homes, supporting comfort, dignity and quality of life under the guidance of RNs and Case Managers.

The schedule runs Friday through Sunday during the day, with travel to assigned service areas and collaboration with the clinical team.

Qualifications

• Current Colorado CNA certification in good standing.
• Hospice, home health or direct patient care experience preferred.
• Valid driver's license and reliable transportation.
• Compassionate, dependable and comfortable providing end-of-life care.

Responsibilities

• Provide personal care including bathing, grooming, dressing and toileting.
• Assist with mobility, transfers, positioning and ambulation.
• Provide assistance with meals, hydration and nutritional needs.
• Observe patients and promptly report changes in condition to the nursing team.
• Help maintain a safe, clean and comfortable environment for patients.
• Travel to patients' homes and other care settings throughout the assigned service area.
• Provide compassionate support while respecting each patient's dignity, privacy and personal preferences.
• Communicate and collaborate with nurses and other members of the interdisciplinary hospice team.
• Build trusting relationships with patients and families during an important time in their lives.

Sangre de Cristo Community Care is seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our hospice home care team in Pueblo, Colorado.

As a Hospice CNA, you'll have the opportunity to provide meaningful, one-on-one care to patients in the comfort of their own homes. You'll be an important part of an interdisciplinary hospice team focused on helping patients live with comfort, dignity and quality of life.

This position works Friday, Saturday and Sunday during the day.

What You'll Do

Working under the direction of an RN or Case Manager and following each patient's individualized Plan of Care, you will:

• Provide personal care including bathing, grooming, dressing and toileting
• Assist with mobility, transfers, positioning and ambulation
• Provide assistance with meals, hydration and nutritional needs
• Observe patients and promptly report changes in condition to the nursing team
• Help maintain a safe, clean and comfortable environment for patients

Requirements
