Job summary

Penn Medicine’s Home Care Chaplain position seeks an experienced chaplain to assess spiritual needs and plan individualized care for seriously ill patients and families. You will document assessments, provide counseling, and coordinate resources with an interdisciplinary team.

The role emphasizes dignity, informed decision-making, and support through grief and end-of-life care. Requirements include a Master’s in a related field, CPE experience, and healthcare/hospice exposure.

Qualifications

• Master of Arts or Science required.
• Education specialization in Divinity, Pastoral Theology, and/or Counseling.
• CPE experience (4 units) preferred.
• At least 1 year experience in a healthcare environment preferred.
• Experience in hospice or home care setting preferred.
• Oncology, geriatric, or hospital chaplain experience preferred.

Responsibilities

• Participate as a member of an Interdisciplinary Team in an individualized plan of care for patients and families.
• Conduct comprehensive spiritual assessments and reassessments and document plans of care.
• Assess spiritual needs and provide counseling to patients and families at risk.
• Develop a care plan with patients and caregivers to address spiritual needs.
• Identify community resources, coordinate spiritual management, and evaluate outcomes.
• Educate patients and families on care factors, disease processes, and grief.
• Demonstrate knowledge of medical terminology and treatment decisions and facilitate advance directives.
• Evaluate adaptation to hospice care and support bereavement needs.
• Identify grief and risk factors; respond with crisis management and seek consultations as needed.

Education

Master of Arts or Science

Education Specialization: Divinity, Pastoral Theology, and/or Counseling
