Job summary

Affinity Hospice Management LLC is seeking a Hospice Chaplain to provide spiritual care to patients and families, coordinating with the care team in Philadelphia, PA.

A Master of Divinity is required; an ordained or commissioned chaplain with CPE experience is preferred. Strong assessment, documentation, and interfaith outreach are essential.

This role participates in IDG meetings, serves as liaison with community chaplains, and may lead funeral services as needed.

Qualifications

• Bachelor's degree in religious studies or related subject and a master's degree in theology, divinity or pastoral studies is required.
• Certification is preferred; ordained or commissioned status is typically required and endorsement from a faith group may be needed.
• Ongoing commitment toward development through education and obtaining CPE.
• Experience in hospice care preferred.
• Comfort in an interfaith setting.
• Ability to network with community clergy and congregations.

Responsibilities

• Assures spiritual assessment of patients and families/caregivers in the hospice program.
• Facilitates development of the individualized plan of care by participating in comprehensive assessment.
• Provides direct spiritual care to patients and families/caregivers.
• Serves as liaison to community chaplains and spiritual counselors.
• Maintains records of spiritual care services for QA and program development.
• Documents direct services and communication with community chaplains.
• Attends patient care conferences as a member of the interdisciplinary group.
• Assists in supervision of spiritual care volunteers as assigned.
