Senior Community Care of Michigan PACE, a program of Volunteers of America National Services, seeks a Registered Dietitian in Lansing, MI. You will provide medical nutrition therapy, perform regular re-assessments, and collaborate with an interdisciplinary team to support nutrition care for older adults.

The role requires RD license in Michigan, 1+ year experience with frail/elderly populations, and a valid driver’s license.

Qualifications

• Bachelor’s or advanced degree from an accredited college with major studies in food and nutrition or dietetics required.
• Licensed in the state as a Dietitian. Registered Dietitian Certification required.
• Minimum of one year of experience working with a frail or elderly population required.
• Must have a valid driver’s license and have means of transportation.

Responsabilités

• Provides quality Nutrition Care following the Nutrition Care Process and Medical Nutrition Therapy.
• Conducts yearly in-person re-assessments and semi-annual re-assessments as needed.
• Participates in Interdisciplinary Team meetings and coordinates care with staff.
• Monitors nutrition-related health metrics and suggests changes to the plan of care.
