Job summary

Senior Community Care of Michigan PACE (a program of Volunteers of America) seeks a Registered Dietitian in Lansing, MI. This role focuses on delivering medical nutrition therapy within an interdisciplinary team, supporting frail and elderly participants, and coordinating care across staff.

Qualifications

• Bachelor’s or advanced degree in Dietetics required.
• Licensed in the state as a Dietitian; Registered Dietitian Certification required.
• Minimum of one year of experience with frail or elderly populations.

Responsibilities

• Provides quality Nutrition Care through the Academy of Nutrition and Dietetics and the Nutrition Care Process, including Assessment, Diagnosis, Interventions, and Monitoring.
• Conducts yearly in-person re-assessments and semi-annual updates as needed.
• Participates in Interdisciplinary Team meetings and coordinates care with staff.
• Monitors monthly weights, diagnoses, nutrient interactions, and hydration status; recommends changes.
