Job summary

Northwestern Medicine is seeking a Home Infusion Clinical Pharmacist to support patient-centered home infusion therapy. The role focuses on clinical monitoring, safety, and collaboration with care teams in an outpatient home infusion setting.

The position requires an active Illinois pharmacist license and a PharmD or Bachelor’s degree. Experience in hospital or home infusion pharmacy is preferred, with on-call responsibilities as part of the schedule.

Qualifications

• Active Illinois Pharmacist license (IDFPR) required.
• Bachelor’s Degree in Pharmacy or Doctor of Pharmacy (PharmD) required.
• Residency or hospital pharmacy experience preferred.

Responsibilities

• Review patient records to determine appropriateness for home infusion therapy.
• Navigate EMR and document progress notes and communications.
• Monitor care plan throughout course of therapy.
• Conduct medication reconciliation and interventions as indicated.
• Provide drug information to health care team and patients; counsel when needed.
• Ensure safe selection of line care supplies for infusion.
• Determine appropriate drug administration methods (IV push, pump, gravity, elastomeric).
• Oversee quality of compounded sterile preparations to ensure USP <797> adherence.
• Provide on-call support per rotating schedule.
