Job summary

University of Chicago Medicine in Calumet City seeks a Clinical Pharmacist to join the Home Infusion Pharmacy, Registry. You will oversee safe, effective drug therapy for infusion in the home or at the infusion site, collaborating with medical providers and insurers to optimize patient outcomes.

The role includes supervising pharmacy technicians, contributing to multidisciplinary care, and supporting staff education to ensure compliance and high-quality pharmaceutical services.

Qualifications

• Graduate of an ACPE accredited College of Pharmacy.
• Doctor of Pharmacy or other advanced degree highly preferred.
• Eligible for licensure as a pharmacist in the state of Illinois within 60 days of employment.
• Completion of an ASHP accredited PGY-1 Pharmacy Residency required or 3+ years hospital staffing experience. Home Infusion experience highly preferred.

Responsibilities

• Maintains compliance with JCAHO, ASHP guidelines, licensing acts, and internal policies.
• Interprets and dispenses drug orders with attention to safety and cost.
• Identifies and resolves potential drug-related problems and suggests rational alternatives.
• Checks orders entered into the system for allergies, duplications, interactions, and dosing accuracy.
• Coordinates clinical and dispensing workflow within the home infusion setting.
• Supervises pharmacy technicians and oversees workflow quality.
• Mentors new staff and participates in ongoing staff education.
• Performs other duties as requested by senior management.
