Job summary

Northwestern Medicine is seeking a Home Infusion Clinical Pharmacist to deliver patient-centered care for home infusion patients, ensuring safe, effective therapy and collaboration with the care team.

The role includes clinical monitoring, medication reconciliation, EMR documentation, and on-call coverage. Requires IL licensure and a PharmD or BPharm; residency or specialty training is preferred.

Qualifications

• Active IL pharmacist license (IDFPR) required.
• Bachelor’s or Doctor of Pharmacy degree.

Responsibilities

• Reviews patient records to determine appropriateness for home infusion therapy.
• Interacts with electronic medical records and documents progress notes.
• Monitors pharmacy care plan throughout course of therapy.
• Performs medication reconciliation and makes interventions as indicated.
• Provides drug information to health care team and patients; counsels when needed.
• Ensures safe selection of line care supplies and appropriate administration methods.
• Manages TPNs in collaboration with the home infusion team.
• Oversees compounding quality and supports cleanroom operations as needed.
• Provides on-call support per rotating schedule.
