Job summary

Qualifications

• Active Illinois Pharmacist license (IDFPR) and PharmD/Bachelor's degree in Pharmacy.
• Completion of a residency or hospital pharmacy experience preferred.

Responsibilities

• Review patient records to determine appropriateness for home infusion therapy.
• Monitor care plans and medication, ensuring patient safety and quality care.
• Provide drug information to health care team and patients; counsel when indicated.
• Oversee sterile preparation quality and assist with equipment and device troubleshooting.
