Qualifications

• CCC-SLP or in progress to certification with supervised experience
• State license to practice as a speech-language pathologist where applicable
• Maintain clinical practice skills and adhere to agency policies
• Must have a valid driver’s license and reliable transportation to work sites

Responsibilities

• Assess, plan, and deliver speech-language pathology services in home health settings
• Develop and revise care plans in collaboration with physicians and clients
• Educate clients and caregivers on treatment and recovery techniques
• Document clinical findings, progress, and updates in accordance with agency policy
• Participate in quality improvement and interdisciplinary coordination
• Supervise HHAs and mentor new staff as assigned
