Job summary

Trinity Health At Home is seeking a Contingent Speech-Language Pathologist to provide professional diagnostic and therapeutic services in home health. You will coordinate care, develop individualized treatment plans, and promote client independence through effective communication.

The role requires CCC-SLP or in-process certification, state licensure, and the ability to work with minimal supervision across multiple sites, with travel as needed.

Qualifications

• CCC-SLP status or in process with ASHA requirements.
• Current state license to practice Speech-Language Pathology.
• Ability to maintain clinical skills and follow agency policies.

Responsibilities

• Assess, plan, and deliver SLP services in home health settings.
• Develop and revise care plans with physicians and clients.
• Educate clients and caregivers on treatment and recovery techniques.
• Document clinical findings and progress per agency policy.
• Participate in quality improvement and interdisciplinary coordination.
• Supervise HHAs and mentor new staff as assigned.
