Job summary

Trinity Health At Home is seeking a Contingent Speech-Language Pathologist to provide professional diagnostic and therapeutic services in home health settings under physician orders at the Monticello-Above and Beyond program. The role focuses on improving communication and swallowing functions, coordinating care with the patient’s care team, and delivering patient-centered services.

This part-time opportunity requires ASHA CCC-SLP credentials or in-progress supervision and an active state

Qualifications

• CCC-SLP from ASHA or in-progress supervised experience.
• Current state license to practice, if applicable.
• Ability to work with limited supervision and maintain professional standards.
• Reliable transportation to multiple work sites.

Responsibilities

• Assess and deliver speech-language pathology services in home health settings.
• Develop and revise care plans in collaboration with physicians and clients.
• Educate clients and caregivers on treatment techniques.
• Document clinical findings, progress, and updates per agency policy.
• Participate in quality improvement and interdisciplinary coordination.
• Supervise HHAs and mentor new staff as assigned.
