• Active RN license required.

• Experience with EMR and case management a plus.
• Ability to work independently within a multidisciplinary team.

Responsibilities

• Perform initial holistic nursing assessment and develop care plans.
• Document Oasis accurately and completely.
• Coordinate and manage patient care in home settings.
• Review medications and provide patient education.
• Complete payer-required forms and referrals.
• Ensure safety and quality of care in the patient environment.
