Job summary

Wilson Health in Sidney, Ohio seeks a Home Health & Hospice Registered Nurse (RN) for a part-time, 1st shift role. The RN will provide in-home skilled nursing, coordinate case management, and educate families to support recovery and independence.

Must hold an Ohio RN license with at least 2 years of nursing experience, preferably in home health or hospice. The role includes travel within the 6-county area and requires strong clinical and communication skills.

Qualifications

• Current and valid RN license in the state of Ohio.
• Minimum of 2 years nursing experience, preferably in home health or hospice.
• Strong communication, organizational and problem-solving skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Provide skilled nursing care to patients in their homes following the care plan.
• Coordinate case management and documentation progress for each client.
• Assess patient safety and develop a comprehensive plan of care.
• Educate patients and families on care plans and medications; complete payer forms.
• Document Oasis assessments accurately and completely.
