Job summary

UnitedHealth Group seeks a Physical Therapist to deliver skilled services in patients' homes, promoting mobility, independence and safety. You will work with patients, families, and an interdisciplinary team to support recovery and prevent hospitalizations.

Responsibilities include regular reexaminations, discharge planning, and coordinating care. A state PT license and CPR certification are required; home health experience is preferred and mentoring is available for those new to home care.

Qualifications

• Current state license to practice physical therapy.
• Graduate from CAPTE-accredited program.
• 1+ year of PT experience in home health preferred; mentoring possible for less experienced.
• Current CPR certification.

Responsibilities

• Provide skilled PT services in patients' homes to promote mobility and independence.
• Conduct periodic reexaminations (at least every 30 days) and discharge planning.
• Collaborate with patients, caregivers, and the interdisciplinary team to support recovery and prevent hospitalizations.
• Other duties as assigned.
