Job summary

Advocate Health at Home is seeking a Part-time Home Health Physical Therapist to manage a patient caseload in the University City area, Charlotte. The role requires a DPT and state license, with ongoing documentation and collaboration with a multidisciplinary team.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current state Physical Therapy license to practice in applicable state (North Carolina/South Carolina).
• BLS for Healthcare Providers certification may be required.

Responsibilities

• Manage daily patient caseload and document care per plan of care.
• Collect data from assessments, home health visits, and EMR.
• Analyze data to identify issues and resources; address high-risk patients.
• Prioritize multi-issue situations based on patient health and safety.
• Evaluate patients to determine rehab needs and plan of care with physician.
• Implement plan of care and supervise PT services and coordinations.
• Maintain communication with the multidisciplinary team and physician orders.
• Incorporate goals into therapy with proper visit notes and milestones.
• Ensure care aligns with PT practice acts and home health guidelines.
