Job summary

Qualifications

• Active Physical Therapy license in the state of practice.
• Current CPR certification.
• Valid driver's license and access to reliable transportation or public transit.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Develops the plan of care and coordinates with the health care team.
• Provides PT services within the scope defined by state practice acts and plan of care.
• Treats patients to relieve pain and restore or maintain function.
• Educates patient and family about plan of care and progress.
• Completes progress notes after visits and documents outcomes.
• Reports patient progress and adjusts care plans as needed.
