Job summary

Trinity Health Mid-Atlantic is seeking a licensed physical therapist to provide high-quality services in patients’ homes through Holy Cross Home Care. You will assess, plan, and implement individualized therapy, aiming to improve independence and life quality while following physician prescriptions.

The role requires Maryland licensure, at least one year of clinical experience (home care preferred), and a valid driver’s license.

Qualifications

• Current licensure as a Physical Therapist in the State of Maryland.
• At least one year of clinical experience (home care experience preferred).
• Graduate of an accredited physical therapy program.
• Valid driver’s license and reliable transportation.
• Ability to work independently and manage time effectively.

Responsibilities

• Provide professional physical therapy services as prescribed by physicians, following state practice acts and organizational policies.
• Assess patient conditions, develop individualized treatment plans, and revise care plans as needed.
• Implement therapeutic and rehabilitative procedures, including manual therapy, exercises, and physical agent modalities.
• Educate patients and caregivers on therapy programs and home exercises.
• Collaborate with interdisciplinary teams to ensure coordinated, effective care.
• Maintain accurate documentation and participate in quality improvement initiatives.
