Job summary

UnitedHealth Group is seeking a Physical Therapist to deliver skilled PT services in patients' homes, promoting mobility, function and safety. You'll work with patients, caregivers and a care team to support recovery and reduce hospitalizations in a home-based setting.

Requirements include a current PT license for the state, CAPTE-accredited education, and CPR certification, with at least 1 year of related PT experience for home health; mentoring possible for less-experienced candidates.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from an accredited physical therapy education program as set by CAPTE.
• 1+ related physical therapy experience for home health PTs.
• If less than one year of experience, regional clinical leadership approval and mentoring/residency participation.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations at minimum every 30 days, modify interventions as needed, and perform discharge planning.
• Collaborate with patients, caregivers and an interdisciplinary care team to support recovery at home.
