Qualifications

• Current license to practice occupational therapy in the assigned state.
• ACOTE-accredited occupational therapy education.
• NBCOT entry-level certification or completion.
• One year related occupational therapy experience (or regional mentoring/ residency approval).
• Current CPR certification.

Responsibilities

• Responsible for all aspects of occupational therapy service delivery provided under a physician approved plan of care.
• Ensures the plan of care addresses all problems identified in the assessment or documents rationale if addressed.
• Accountable for the safety and effectiveness of the occupational therapy service delivery process.
• Completes documentation timely and accurately.
• Supervises home health aide when RN is not active in the case.
• Other duties as assigned.
