BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide in-home visits in Harford County, Maryland. This part-time or per-diem role offers flexibility to support adult and geriatric patients in their homes.

The OT will assess function, cognition, and safety, develop individualized plans, and coordinate with a multidisciplinary team. Prior home health experience is preferred, with strong documentation and EMR skills valued.

Qualifikationen

• Current MD Occupational Therapist license.
• Home health experience preferred, but not required.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an OT program accredited by AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Aufgaben

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects and develop intervention plans.
• Evaluate home environment for hazards and need for adaptations.
• Identify equipment needs and environmental adaptations.
• Revise care plans and collaborate with nursing in the multidisciplinary team.
• Educate patients and family in rehabilitative care and safety for independent living.
• Document observations and interventions using an EMR tablet.
