Job summary

BAYADA Home Health Care in Annandale, Virginia, seeks a part-time Occupational Therapist to perform home health visits across Fairfax County. You will evaluate patients, develop individualized plans, and coordinate with nursing and therapy teammates to support rehab goals in the home.

As a member of a compassionate, patient-centered team, you will document care using a state-of-the-art EMR, educate patients and families, and help ensure safety and independence at home.

Qualifications

• Virginia OT license required.
• Graduate from an accredited OT program.
• Minimum one year OT experience preferred.
• Ability to work independently and collaborate with team.
• Strong communication and organizational skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status and ADL/IADL.
• Develop and adjust individualized intervention plans.
• Evaluate home environments for hazards and independence.
• Educate patients and families on rehabilitative activities.
• Document observations and care using EMR.
