Job summary

BAYADA Home Health Care is seeking an Occupational Therapist to provide home health visits in Falls Church, Virginia. This part-time role serves adult and geriatric patients in Fairfax County, VA, with duties including evaluations, care plan development, and education to promote the client's health and independence.

Qualifications

• Current Virginia Occupational Therapist license.
• Minimum of one year of OT experience.
• Graduation from an OT program accredited by AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal and communication skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assess functional status, ADL/IADL performance, and cognitive/motor aspects.
• Develop and revise individualized OT care plans to achieve outcomes.
• Evaluate home environments for hazards and adaptations.
• Identify equipment needs and environmental modifications.
• Document observations and interventions in EMR.
