Job summary

BAYADA Home Health Care in Warrenton, VA seeks an experienced Occupational Therapist (OT) for home health visits in adult and geriatric populations. The role supports per visit, part-time, or full-time schedules across senior living communities within the Warrenton area.

As part of a multidisciplinary team, you will assess function, develop plans, educate patients and families, and document care using a touchpad EMR. Prior home health and VA licensure are preferred.

Qualifications

• Must hold VA OT license.
• At least 1 year recent OT experience required.
• Graduation from an OT program accredited by AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Perform thorough evaluations of functional status and ADL/IADL performance.
• Develop individualized intervention plans to achieve outcomes.
• Evaluate home environment for hazards/barriers to independence.
• Identify equipment needs and environmental adaptations.
• Document observations and interventions using an EMR system.
