Job summary

The Lakes Home Care Services is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes according to an established plan of care. The role emphasizes safety, documentation, and collaboration with nursing staff.

Responsibilities include assisting with activities of daily living, monitoring vital signs, and ensuring patient comfort while maintaining a respectful, supportive environment.

Qualifications

• Meets training requirements under state and federal laws where applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintains current CPR certification as required by state regulations.

Responsibilities

• Assist patients with daily living activities per plan of care (baths, oral hygiene, dressing, toileting, feeding, etc.).
• Record vital signs when ordered and report changes to nurse supervisor.
• Assist with safe transfers and ambulation and perform ROM as ordered.
• Support patient safety and home safety practices; observe and document patient condition.
