Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to provide daily living assistance and personal care to clients in their homes according to individualized care plans. You will support with bathing, grooming, mobility, and routine tasks while ensuring safety and dignity.

This is a part-time role offering flexible hours. You will follow the care plan, monitor patient condition, document changes, and communicate with the supervising nurse.

Qualifications

• CPR certification current and valid.
• Ability to read and follow written instructions.
• Strong communication with clients and families.
• Completion of Maxim competency program.

Responsibilities

• Assist patients with daily living per plan of care (baths, hygiene, dressing, Toileting).
• Take and record vital signs when ordered (temp, pulse, resp, BP).
• Perform safe transfers and ambulation as directed.
• Support exercises per plan of care and assist with ROM.
• Handle patient laundry and maintain safety with equipment.
• Observe safety practices and report changes to nurse supervisor.
