Job summary

The Lakes Home Care is seeking a compassionate Home Health Aide to provide daily living and personal care for patients in their homes, following an established plan of care. You will assist with baths, dressing, grooming, toileting, transfers, and observation.

Part-time position with flexible hours and a supportive team. Must have CPR certification, read and follow written instructions, and complete Maxim screenings and competency program.

Qualifications

• Reading and following written instructions is required.
• CPR certification must be current and maintained.
• Completion of Maxim screening examinations and competency program.

Responsibilities

• Assist with baths, dressing, grooming, toileting and feeding as per care plan.
• Take and record vital signs when ordered and report changes.
• Support safe transfers and ambulation according to instructions.
• Perform range of motion and simple procedures as directed.
• Ensure patient safety and observe home safety practices.
• Document care given and communicate with nurse supervisor.
