Job summary

The Lakes Home Care is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes according to an established care plan.

Responsibilities include assisting with baths, hygiene, transfers, and daily activities, while documenting patient status and reporting changes to the supervising nurse. Must meet state requirements, maintain CPR certification, and follow the care plan with clear communication.

Qualifications

• Meets training requirements per state/federal laws where applicable.
• Able to read and follow written instructions and document care.
• Maintain current CPR certification as required by state regulation.
• PPD or chest X-ray if applicable per policy.

Responsibilities

• Assist patients with daily living tasks per plan of care (baths, dressing, toileting, feeding).
• Take and record temperatures, pulse, respiration, and blood pressure when ordered.
• Ensure safe transfers and ambulation; perform range of motion exercises as directed.
• Observe and document patient condition; report changes to nurse supervisor.
• Follow care plan and complete assigned tasks; communicate clearly with client and family.
