Job summary

The Lakes Home Care is seeking a compassionate Home Health Aide to provide daily living support and personal care in patients' homes according to an established care plan. Responsibilities include assisting with bathing, feeding, dressing, and activities of daily living, while documenting progress and communicating with the nurse supervisor.

Requirements include current CPR certification, ability to read and follow care plans, and completion of competency programs.

Qualifications

• CPR certification up-to-date as per state regulations.
• Able to read and follow written care plans.
• Submit care documentation accurately and timely.
• Strong communication with clients, families, and supervisors.

Responsibilities

• Assist patients with activities of daily living as per care plan.
• Help with baths, oral hygiene, dressing, toileting, feeding, and related tasks.
• Record measurements and vital signs when ordered and report changes.
• Assist with safe transfers and ambulation as directed.
• Perform routine exercises and simple therapy activities per plan.
• Handle patient laundry and maintain safety in the home.
• Observe, document, and report patient condition changes to nurse supervisor.
