Job summary

The Lakes Home Care is seeking a compassionate Home Health Aide to support patients in their homes with daily living and personal care according to an established care plan.

You will assist with bathing, grooming, mobility, and feeding, while documenting patient condition and communicating with the assigned nurse. This part-time role offers flexible hours and opportunities to deliver quality in-home care.

Qualifications

• Meets training requirements as per state and federal laws
• Able to read, follow written instructions and document care
• Maintains CPR certification as required by state regulation
• Completes Maxim screening examinations and competency program

Responsibilities

• Assist patients with daily living activities per care plan
• Provide baths, oral hygiene, dressing, toileting, feeding
• Record vital signs when ordered and report changes to nurse
• Ensure safe transfers, ambulation, and activity according to plan of care
• Observe home safety practices and document patient condition
