Job summary

The Lakes Home Care is seeking a compassionate Home Health Aide to provide daily living and personal care in patients' homes according to an established care plan. Responsibilities include assisting with baths, dressing, feeding, taking vital signs, safe transfers, ROM, observation, and reporting to the supervising nurse; you will follow the care plan and communicate clearly with clients and families.

Qualifications

• Must meet training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Possess and maintain CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Assist patients with the plan of care including baths, oral hygiene, dressing, toileting, feeding.
• Record and monitor vital signs and report changes to the supervising nurse.
• Perform safe transfers and ambulation, ROM and other extensional therapy as ordered.
• Handle patient laundry and maintain safety practices using equipment properly.
• Observe, document, and communicate patient condition; follow up with the client and family.
