Job summary

UnitedHealth Group seeks an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members. The role focuses on education, gap closure, and coordination with primary care providers to improve outcomes.

You will work in patients’ homes, addressing social determinants of health, providing referrals, and ensuring safe, evidence-based care with a flexible schedule and balanced workload.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state with start-date ability.
• Active and unrestricted driver's license with access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• If applicable, eligibility to practice independently and timelines to obtain approvals.

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Identify and document diagnoses for care management and treatment planning.
• Communicate findings to members' PCPs to address gaps in care.
• Recognize urgent/emergent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
