Job summary
Amg-Senior-Medical-Group seeks a Nurse Practitioner to provide home-based primary and palliative care across the assigned geographic area. You will perform comprehensive medical evaluations, work with patients and families to create personalized care plans, and focus on comfort and quality of life for elderly, chronically ill, and homebound individuals.
Responsibilities include EMR documentation within 72 hours of visits, coordinating with a multidisciplinary team, educating families about care
Qualifications
- Must hold a Master’s or Doctorate in Nursing Practice and an active NP license.
- Current CPR certification and a valid DEA license.
Responsibilities
- Provide home-based primary and palliative care visits in homes and facilities.
- Perform comprehensive medical evaluations and create personalized care plans.
- Manage EMR documentation within 72 hours of visits.
- Coordinate with multidisciplinary teams to ensure holistic care.
- Educate patients and families about care options and goals.
- Facilitate access to hospice and community resources.
- Provide coverage for other practitioners as needed.
