Signature Healthcare at Home is seeking a compassionate Part-time Nurse Practitioner to deliver comprehensive patient care in the home health and hospice setting. Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Current Nurse Practitioner licensure in State and BLS Certification.
• Master’s degree with a minimum of one year Nurse Practitioner experience.
• Accreditation from an approved certifying body for advanced practice nursing as required by State.

Responsibilities

• Provide medical care in compliance with the state Nurse Practice Act as assigned and in consultation with Department Directors.
• Develop an individualized plan of care, prescribe medications and/or treatments, obtain consultations or referrals and coordinate health care services.
• Complete face to face visit requirements as assigned based on conditions of participation and local coverage determinations.
• Educate/counsel patients, families, and/or caregivers on preventive care and health concerns in conjunction with the care team.
