Heritage Communities in Omaha, NE is hiring a Health Services Coordinator at The Heritage at Sterling Ridge to support resident care and regulatory readiness. You will assess residents, coordinate caregiver training, and collaborate with families, health partners, and staff to enhance quality of life.

The role requires an active LPN license, CPR/BLS, and 1+ years in a supervisory capacity within senior living.

Qualifications

• Active state nursing licensure (LPN) with good standing.
• Current CPR/BLS certification.
• 1+ year supervisory/leadership experience in senior living.
• Experience caring for cognitive/memory-care residents preferred.

Responsibilities

• Conduct resident assessments pre-move-in and ongoing.
• Support regulatory readiness with accurate care plans and records.
• Build relationships with residents, families, and partners.
• Coordinate caregiver training and evaluate care delivery.
• Provide direct resident care and medication administration as ordered.
