Job summary

Heritage Senior Communities is seeking a Resident Services Coordinator to oversee resident services, care plan development, and collaboration with clinical and operational teams. The role emphasizes high-quality care for frail seniors, including those with dementia, with oversight of medication and documentation systems.

Ideal candidates have LPN credentials or equivalent, Michigan licensure, CPR/First-aid, strong leadership, and excellent communication.

Qualifications

• Experience with frail elderly and dementia care.
• Must maintain Michigan licensure/registration and CPR/First-aid certifications.
• Course work or experience in gerontology or geriatrics.
• Combined nursing and management experience.
• Strong communication skills, verbal and written (email).
• Leadership and supervision of staff; ability to educate and motivate.

Responsibilities

• Coordinate Personal Service Assessments and develop care plans within guidelines.
• Oversee medication administration program and medical records documentation.
• Coordinate with outside providers (podiatry, pharmacy, home health, PT, etc.).
• Visit residents in hospital/SNF to ensure transition back to living center.
• Ensure diet orders are communicated to Culinary Service Department.
• Maintain compliance with licensing, HPM policies, and reporting requirements.
• Lead weekly Safety Review and quarterly Infection Prevention activities.
• Oversee staff scheduling and performance evaluations.
