Job summary

MBK Senior Living is seeking a part-time Health Services Coordinator (LPN) to join our Sky Vista community, supporting the health services team with administrative and clinical tasks under the Director of Health Services. Shift: Fri-Sun or Sat-Mon, 9am-5pm.

You will manage initial assessments, draft and update service plans, coordinate medications, ensure accurate documentation, and promote resident dignity and independence in line with MBK policy.

Qualifications

• Active RN, LVN or LPN license in good standing.
• 2+ years of related healthcare experience.
• Must complete background clearances, health screening, and TB test within required timeframe.
• Valid driver’s license and reliable transportation.
• Proficiency with Microsoft Office and basic computer skills.

Responsibilities

• Conduct and coordinate assessments of potential residents and make admission recommendations.
• Draft initial individualized Service Plans and update as needed.
• Coordinate prescription orders with doctors and manage pharmacy deliveries.
• Provide training on medication delivery, safety protocols, and state requirements.
• Maintain accurate documentation and required records.
