Job summary

MBK Senior Living is seeking a Health Services Coordinator (LPN/LVN/RN) to support the health services team and assist in managing the overall operations of the department. You will conduct resident assessments, draft and review care plans, and coordinate medications under the guidance of the Director of Health Services.

Experience in a similar healthcare setting is required, with strong communication skills and the ability to work in a fast-paced, team-oriented environment.

Qualifications

• RN, LVN or LPN license that is active and in good standing is essential; adherence to all requirements to maintain license including CEU completion and timely renewal.
• 2+ years of prior related work experience in a healthcare environment.
• Must complete required background clearances, health screening and provide negative TB test results within 7 days of employment (last 6 months).
• Valid state driver’s license and valid insurance or reliable transportation.
• Excellent communication skills including the ability to speak, write and read English.

Responsibilities

• Conduct and coordinate assessments/evaluations of potential residents and make admission recommendations in accordance with rules and policies.
• Draft initial individualized Service Plans and update as needed.
• Review service plans with responsible parties and the Executive Director as requested by DHS.
• Provide training on medications, safety protocols, and emergency procedures; ensure accurate records per state regulations.
• Perform basic nursing functions including treatments and medication delivery; respond to resident emergencies.
