Job summary

Bond Health is seeking a part-time Registered Nurse to manage nursing care for individuals in a residential group home. The role includes coordinating medical care, monitoring wellness, and ensuring regulatory compliance while collaborating with physicians and care teams.

The RN will maintain care plans, oversee medication administration, complete clinical documentation, and participate in quality improvement activities. NY licensure and CPR certification are required or preferred as specified.

Qualifications

• Current unrestricted New York State Registered Nurse (RN) license.
• Current CPR certification.
• Experience with individuals with intellectual and developmental disabilities is preferred but not required.

Responsibilities

• Coordinate medical care and monitor health status of assigned individuals.
• Assess, document, and follow up on changes in physical condition and overall wellness.
• Communicate with physicians, families, interdisciplinary team members, and other healthcare providers.
• Monitor hospitalizations, ER visits, injuries, illnesses, and follow-up care.
• Attend medical appointments to advocate for individuals served.
• Develop, update, and maintain individualized nursing care plans and protocols.
• Oversee medication administration and ensure compliance with medication management procedures.
• Review medical records, MARs, treatment logs, and related documentation.
• Monitor dietary and nutritional needs and provide guidance to support staff.
• Complete nursing assessments, progress notes, and required clinical documentation.
• Maintain infection control standards and ensure medical supplies are available.
• Participate in quality improvement activities, meetings, and required training.
• Perform other duties as assigned.
