Job summary

Bond Health in Brooklyn is looking for a Registered Nurse for a full-time position to support individuals completing CRTP and Transitional Respite programs. The RN will perform initial and ongoing assessments, manage medications, and coordinate medical and behavioral health needs with a multidisciplinary team.

The role requires a New York State RN license and at least 2 years of nursing experience, with preferred background in IDD, behavioral health, crisis care, or community health.

Qualifications

• Current New York State RN license.
• Minimum 2 years of nursing experience; IDD, behavioral health, crisis care, or community health background preferred.
• Familiarity with OPWDD, OMH, and DOH nursing regulations and program requirements.
• Strong clinical assessment, crisis intervention, and care coordination skills.
• Excellent communication, organizational, and documentation skills.
• Ability to work independently while thriving in a team-based environment.

Responsibilities

• Screen individuals admitted to CRTP and participate on the intake team.
• Conduct initial and ongoing nursing assessments for admitted individuals.
• Oversee self-medication administration; monitor response and document outcomes.
• Provide direct nursing care including wound care, diabetes management, and respiratory support.
• Monitor for medical or psychiatric crises and initiate appropriate interventions.
• Review medication administration records and vital signs logs; maintain records.
• Coordinate with clinicians regarding medications, renewals, and drug information sheets.
• Manage medication storage and disposal per federal and state law.
• Develop and update health goals reflecting current status of the person supported.
• Ensure infection control and universal precautions per OSHA guidelines.
• Order and maintain first aid and medical supplies; be available 24/7 by phone.
• Partner with ICM teams to address medical and behavioral health needs in service planning.
• Deliver health education and coaching to individuals, families, and direct support staff.
• Facilitate hospital discharge and community reintegration through CTI for continuity of care.
• Coordinate follow-up care with primary care providers, specialists, and community resources.
• Serve as clinical liaison among providers, families, and program staff.
• Participate in interdisciplinary treatment planning and case conferences.
• Train and mentor direct support professionals on health tasks and emergency response.
