Job summary

Pine Park Health is seeking a Part-time Nurse Practitioner to deliver geriatric primary care in senior living communities around the SF Bay Area. You will collaborate with a Medical Director and dedicated RNs to provide on-site visits, care plans, and CCM visits, while shaping clinical protocols in a value-based model.

Qualifications

• Nurse Practitioner certification with geriatric care experience.
• Willingness to provide onsite clinical services across multiple communities.

Responsibilities

• Provide onsite primary care services to older adults in Senior Living Communities.
• Develop and execute detailed person-centered care plans for all primary care patients.
• Work with Medical Director to identify and develop policies addressing clinical issues and culture.
• Provide monthly Chronic Care Management (CCM) visits and documentation.
• Collaborate with senior living management to improve patient outcomes and care delivery.
• Contribute to clinical innovation and quality improvement initiatives.
