Job summary

Pine Park Health is seeking a Nurse Practitioner to deliver onsite primary care for older adults in East Bay Senior Living Communities. You will craft detailed care plans and work closely with a multidisciplinary team to provide holistic, value-based care.

Ideal candidates have geriatrics experience, strong communication, and willingness to travel within the East Bay region. The role supports a hybrid work model with emphasis on collaborative, patient-centered care.

Qualifications

• Nurse Practitioner with geriatrics experience preferred.
• Willingness to provide onsite clinical across the East Bay San Francisco region.
• Strong written and verbal communication skills.
• Comfortable in a fast-paced, changing environment.
• Willingness to travel between East Bay partner communities.

Responsibilities

• Provide onsite primary care services to older adults in Senior Living Communities.
• Develop and execute detailed person-centered care plans for primary care patients.
• Collaborate with Medical Director to identify and develop policies affecting practice and guidelines.
• Provide CCM visits and document CCM activities.
• Lead regular huddles and mentor the clinical team.
• Coordinate with senior living management to improve Pine Park care plans and outcomes.
