Job summary

Spectrum Medical Partners is seeking a part-time Behavioral Health Case Manager to provide evidence-based psychotherapy and behavioral health services within post-acute and long-term care facilities in Florida. The role emphasizes crisis response, care coordination, and adherence to regulatory standards while supporting value-based care and patient well-being.

You will collaborate with facility teams, conduct assessments, and maintain accurate EHR documentation.

Qualifications

• Doctoral or master's degree in psychology, counseling, or social work.
• Active Florida independent license (LCSW or LMHC).
• Minimum of two years of clinical experience in behavioral health.
• Experience with geriatric or medically complex populations preferred.
• Proficiency in evidence-based therapies (CBT, supportive, solution-focused).

Responsibilities

• Provide psychotherapy using evidence-based interventions for post-acute and long-term care residents.
• Deliver behavioral interventions to address mood, cognitive, and behavioral symptoms.
• Conduct psychological and behavioral assessments to support diagnosis and treatment goals.
• Collaborate with physicians, nursing, therapy, social services, and interdisciplinary teams.
• Coordinate with internal PHA providers to support continuity of psychiatric and psychological care.
• Complete accurate, timely documentation within the EHR per policies and regulatory standards.
• Follow AHCA, DCF, CMS, and Florida practice guidelines; maintain HIPAA compliance.
• Provide crisis assessment and intervention including risk management and Baker Act protocols when indicated.
• Educate facility staff to enhance understanding of mental health conditions and care approaches.
• Maintain licensure and pursue continuing education; stay current with evidence-based practices.
