Job summary

Spectrum Medical Partners seeks an independently licensed Behavioral Health Case Manager to deliver evidence-based psychotherapy and behavioral interventions in post-acute and long-term care facilities. You will conduct psychological assessments, coordinate with interdisciplinary teams, and ensure compliant documentation and crisis response.

Travel to facilities and home-office work are required. Required licensure (LCSW or LMHC) in Florida and a minimum of two years of behavioral health

Qualifications

• Doctoral or master’s degree in psychology, counseling, or social work from an accredited institution.
• Active Florida independent license as LCSW or LMHC.
• Minimum of two years of clinical experience in behavioral health.

Responsibilities

• Provide psychotherapy using evidence-based interventions for post-acute and long-term care residents.
• Deliver behavior therapy and supportive interventions for mood, cognitive, and behavioral symptoms.
• Conduct psychological and behavioral assessments to support diagnosis and care planning.
• Collaborate with physicians, nursing, therapy, social services, and interdisciplinary team members for integrated care.
• Complete accurate documentation within the EHR per policies and regulatory standards.
• Provide crisis assessment and intervention and follow risk management procedures.
