Job summary

Sweetwater Care in California seeks an LPN to provide nursing care within scope of practice, follow physician orders, and support center policies. You will participate in infection control, quality improvement, and patient education, with rounds alongside physicians when needed.

Candidates must hold an active LPN license in the state; prior management experience is a plus, and long-term care experience is preferred. Proficiency with PointClickCare charting is a plus.

Qualifications

• Current LPN license in the state where the facility operates.
• One-year nursing management experience preferred, not required.
• Long term care experience preferred.

Responsibilities

• Provides nursing care within scope of practice per standards, physician orders, and center policies.
• Participates in infection control and quality improvement programs.
• Uses the Quality Life Manual for patient identification and care placement in programs.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Educates patients and families about care plans.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives/transcribes/implements physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
• One-year prior nursing management experience preferred, not required.
