Job summary

Sweetwater Care in California is seeking an LPN with management experience to oversee nursing staff within our long-term care facility. You will ensure care meets standards, follow physician orders, and participate in quality initiatives.

The ideal candidate has an active California LPN license, familiarity with Long Term Care and PointClickCare charting, and strong leadership and education skills to support both staff and residents.

Qualifications

• Licensed LPN in California.
• Experience in long-term care.
• Nursing management experience preferred but not required.

Responsibilities

• Follow infection control procedures.
• Participate in quality improvement program.
• Utilize the Quality Life Manual for patient placement and monitoring.
• Notify departments of patient changes (room changes, LOA, admit, discharge).
• Conduct family and patient education.
• Administer medications timely per facility policy.
• Administer treatments timely per facility policy.
• Transcribe and carry out physician orders as allowed by Nurse Practice Act.
• Make rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
• One-year prior nursing management experience preferred, not required.
