Job summary

Sweetwater Care in Davis, CA seeks a licensed practical nurse to provide nursing care within the scope of practice and in accordance with physician orders and facility policies.

The role requires timely administration of medications and treatments, patient education, and collaboration with the healthcare team. Prior long-term care experience and familiarity with Point Click Care are preferred. This is a full-time position based in our Davis facility.

Qualifications

• Licensed LPN in California.
• One year of nursing management experience preferred.
• Long-term care experience preferred.

Responsibilities

• Provide nursing care within scope of practice following physician orders and facility policies.
• Monitor and document patient status, notify changes to other departments as needed.
• Educate patients and families about care plans.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LPN in state
• One-year prior nursing management experience preferred, not required.
