Job summary

UPMC Altoona is seeking a part-time Discharge Planning Associate to join the Clinical Care Coordination and Discharge Planning team. The role is 20 hours per week, Monday through Friday daylight hours, with weekend/holiday rotation.

Responsibilities include identifying factors shaping discharge plans, coordinating care, and ensuring timely post-hospital arrangements while advocating for patient safety and regulatory compliance.

Qualifications

• Diploma or associate degree in nursing with active RN license.
• Bachelor's degree in social work or related health field for Social Work track.
• 1 year (or more) discharge planning experience preferred for Senior/Manager tracks.

Responsibilities

• Identify factors shaping the discharge plan.
• Lead interdisciplinary teams for discharge execution.
• Coordinate post-hospital arrangements to avoid delays.
• Communicate with patients and care teams to build individualized plans.
• Advocate for patient safety and regulatory compliance.
