Qualifications

• High School Diploma or equivalent required.
• Experience in discharge planning or hospital setting preferred.
• Prior authorization experience valued.
• Patient care experience in a hospital setting preferred.
• Excellent communication and strong organizational skills required.

Responsibilities

• Performs clerical tasks delegated by Discharge Planning staff (copying, faxing).
• Provides forms and resources to patients/caregivers for discharge planning.
• Uses EMR and technology to conduct chart reviews and document progress.
• Serves as a contact between patients and discharge planning staff.
• Gather and arrange materials to enable patient discharge; order supplies as needed.
• Complies with HIPAA and system-wide competencies.
• Reports concerns to the Discharge Planning team promptly and clearly.
• Assists with questions and troubleshooting related to discharge planning.
